Job Summary
Provide in-home case management for up to 9 families under the KSTEP program in Kentucky. Responsibilities include conducting assessments, implementing evidence-based treatment modalities with safety planning in clients' homes, facilitating referrals to treatment and resources, delivering direct therapeutic support for up to 8 months per family, maintaining case notes and Credible data entry, reporting weekly to DCBS, coordinating family team meetings, developing timely Action and Treatment Plans, ensuring confidentiality, participating in on-call schedules, assisting with resource access to reduce family stress, and discharge planning. Requires a Bachelor's degree in a human services field, experience with multi-problem families, crisis intervention familiarity, ability to set goals, and the capacity to network with community stakeholders and providers. Position serves multiple Kentucky counties and offers a full benefits package with competitive pay; travel within the region may be involved.
Required Qualifications
- Bachelor’s Degree in a human services field (social work, counseling, psychology, etc.) from an accredited school
- Experience working with multi-problem families
- Familiarity with crisis interventions
- Ability to develop behaviorally specific goals
- Ability to teach necessary skills
- Willingness to engage in in-home case management
- Compliance with confidentiality requirements
