Job Summary
Advise clients and family members on care options within Amsta, evaluate care requests, and coordinate the registration, waiting list, and placement process. Match clients with appropriate care locations and manage indications for WLZ, ZVW, and WMO, while arranging bridging or emergency placements. Coordinate with care facilities and professionals to ensure clear communication throughout the journey from registration to placement. Signal and improve bottlenecks within the system. Work 24 to 36 hours per week in Amsterdam, with partial remote work possible after onboarding.
Required Qualifications
- een relevante opleiding afgerond, zoals Gezondheidswetenschappen, Verpleegkunde, Fysiotherapie, Ergotherapie of een vergelijkbare studie
- kennis van het Nederlandse zorgstelsel en de verschillende zorgwetten (WLZ, ZVW en WMO)
- ervaring in de zorg of een zorggerelateerde functie
- affiniteit met zorgbemiddeling, cliëntadvies en complexe plaatsingsvraagstukken
- affiniteit met ouderenzorg en complexe zorgvragen
- kennis van het Amsterdamse zorgnetwerk
- het aanleveren van een Verklaring Omtrent Gedrag (VOG)
- het opvragen van een referentie
