Job Summary
Accept and respond to referrals from the community, kinship parents, and inter-agencies to provide effective care to children. Create a Navigation Plan and needs assessment in partnership with the caregiver to meet identified needs, while obtaining necessary releases and consents as required. Maintain frequent communication with your supervisor regarding services delivered and concerns, and assist caregivers in identifying barriers to receiving services. Collect and record all data on presenting problems and resources in the client database system, then analyze data to identify trends for future systemic advocacy. Act as a liaison with key community stakeholders, attend community meetings, and develop professional knowledge through in-service training sessions and conferences. Complete 20 hours of annual training and other projects as needed.
Required Qualifications
- Bachelor's degree in social work or related human services field
- Two or more years of experience in child welfare, family advocacy, case management, or closely related area
- Valid driver's license
- Ability to operate a motor vehicle
- Excellent written and oral communication skills
- Ability to handle confidential information
- Ability to manage administrative details
- Ability to work independently
- Ability to prioritize work
- Ability to meet deadlines
- Ability to develop trust-based relationships with children, caregivers, and co-workers
