Job Summary
Conduct case management for kinship foster families by identifying needs, advocating for community resources, and facilitating stability through home visits, telehealth, and phone contact. Serve as a liaison between families and state agencies while transporting clients to community locations and maintaining clinical records. Collaborate with OKDHS personnel to support placement permanency and promote a trauma-informed organizational culture. Complete required training, maintain professional certification, and adhere to confidentiality policies and agency procedures.
Required Qualifications
- Bachelor's Degree in Social Work or related field
- Master's Degree in a closely related field
- Minimum of one (1) year experience in a related field
- Proficiency in the MS 365 applications including Word, Excel, Teams, and Outlook
- Intermediate general computer skills
- Good interpersonal skills and the ability to effectively communicate verbally and in writing with diverse community members, employees, and other stakeholders
- Ability to work collaboratively and maintain productive relationships with the community, employees, and other stakeholders
- Ability to bend, stoop, use stairs, and move about facilities and community freely
- Casual professional dress
- Access to reliable transportation
- In state travel
- Valid Oklahoma driver's license
- Current automobile liability insurance including uninsured motorist coverage
- Must complete all NorthCare required training within 30 days of employment
- Must have TB test within 12 months prior to employment with NorthCare or obtain within 30 days of employment with NorthCare
- Adherence to any and all laws, professional standards and ethical codes that apply to his/her job responsibilities
