Kidney Care Specialist

Job Summary
Provide virtual care navigation and education to CKD and ESRD patients regarding disease progression, treatment options (including home modalities and transplantation), and self-management strategies using approved digital platforms. Conduct scheduled and on-demand virtual or in-person patient interactions (video, phone, and electronic communication) to assess patient needs, readiness, barriers, and preferences. Collaborate with nephrologists and care teams to support coordinated patient care and referrals. Build and maintain relationships with practice staff to support Kidney Care Advocate objectives. Facilitate engaging, inclusive virtual group sessions and follow-up to reinforce education and ensure patient engagement across the kidney care continuum.
Required Qualifications
- High School diploma or G.E.D. required
- 2-5 years clinical background preferred
- 2-5 years’ experience in group facilitation, coaching, counseling, patient support, education, or related fields
- Prior experience working with individuals managing chronic conditions is preferred
