Job Summary
Field-based Kidney Care Specialist role focusing on patient education, care navigation, and engagement for CKD/ESRD patients. Collaborates with nephrology providers and care teams to deliver timely education, coordinate referrals, and support patients through the kidney care continuum using virtual, in-person, or hybrid modalities. Requires strong communication, ability to educate patients remotely, experience with digital platforms/CRM/EMR systems, and willingness to travel within the assigned market.
Required Qualifications
- High School diploma or G.E.D. required
- 3-5 years clinical background preferred
- Relevant licensure or certification preferred, as applicable
- Experience in kidney care, CKD, ESRD, dialysis, or chronic disease management preferred
- Prior experience delivering virtual or telephonic patient education is strongly preferred
- Strong communication and patient education skills
- Ability to engage patients remotely and build trust in a virtual environment
- Proficiency with CRM systems (e.g., Salesforce) and/or EMRs
- Comfort with digital platforms and virtual meeting tools
- Valid driver’s license with reliable transportation and ability to travel within the assigned market
