Job Summary
Kidney Care Engagement Specialist delivers empathetic, virtual one-to-one support and facilitates small-group discussions to help CKD/ESRD patients understand kidney care pathways, education, and treatment options. Responsibilities include outbound/inbound outreach, website chat management, supportive phone conversations, virtual group facilitation (45–60 minutes), and follow-up to ensure care coordination and patient readiness. Collaborates with referral partners and care teams, maintains patient engagement and documentation in CRM/EMR, supports referral workflows, and uses digital platforms to track outcomes. Requires strong virtual facilitation, clear communication, knowledge of kidney disease and treatment modalities (including home therapies), CRM proficiency (e.g., Salesforce), Microsoft 365, and the ability to work remotely with occasional travel; High School diploma or GED required; 2-5 years clinical background preferred.
Required Qualifications
- High School diploma or GED required
- 2-5 years clinical background preferred
- 2-5 years’ experience in group facilitation, coaching, counseling, patient support, education, or related fields preferred
- Prior experience with individuals managing chronic conditions preferred
- College degree preferred
