Job Summary
Drive virtual care navigation and education for CKD/ESRD patients, conducting virtual, in-person, or hybrid patient interactions to assess needs and support treatment choices. Collaborate with nephrologists, advanced practice providers, and care teams to ensure coordinated patient care, referrals, and documentation in CRM/EMR. Utilize digital platforms to track engagement and outcomes, and participate in ongoing development of virtual education materials. Travel within assigned market as needed to support field-based and hybrid roles, while maintaining strong communication with referring practices and internal stakeholders. Demonstrate strong patient-facing communication, organizational skills, and proficiency with CRM systems and virtual meeting tools to deliver patient-centered kidney care education and navigation.
Required Qualifications
- High School diploma or G.E.D. required; 3-5 years clinical background preferred
