Job Summary
Kidney Care Specialist provides field-based, virtual, or hybrid patient education and care navigation for CKD/ESRD patients. Responsibilities include virtual and in-person interactions (video, phone, and electronic), education on disease progression and treatment options (including home modalities and transplantation), collaboration with nephrologists and care teams, documentation in Salesforce/EMR, supporting referral workflows, and using dashboards to track engagement and program performance. Strong communication, organization, and ability to work autonomously yet collaboratively are required; proficiency with CRM systems and digital platforms is preferred.
Required Qualifications
- High School diploma or G.E.D. required; 3-5 years clinical background preferred
