Job Summary
Conduct client outreach, assessments, and individualized service planning for pregnant women and mothers up to 60 days postpartum experiencing moderate to severe substance use disorders. Provide education on recovery, treatment options, and support services while coordinating care and making referrals to appropriate agencies. Serve as the primary contact for referrals from local health departments and collaborate with community partners, including courts, DCBS, law enforcement, and shelters. Maintain accurate documentation and case records while supporting clients in navigating complex systems. This full-time role requires a Bachelor's degree in a behavioral science field and at least one year of case management experience. DBH-approved Targeted Case Management training is provided within six months of hire.
Required Qualifications
- Bachelor's degree in a behavioral science field
- Bachelor's degree in a social or behavioral science field
- At least one (1) year of experience providing case management services
- Ability to complete required training within six (6) months of hire
- Experience working with high-risk or underserved populations
- Strong organizational and communication skills
- Ability to manage a caseload independently while collaborating with a team
- Passion for supporting maternal health and recovery services
- This is a full time position
