Job Summary
RN Care Manager in acute care with responsibilities including screening for post-acute needs, transition planning, care coordination, education, community care referrals, and adherence to regulatory requirements. Role supports multidisciplinary collaboration, ongoing monitoring of care plans, and reassessment of CM needs; minimum 2 years acute care/community experience; BSN preferred; OR RN license required; part-time and full-time shifts available. Required Qualifications
- Nursing degree/diploma required upon hire
- Bachelor's degree in Nursing or higher within 3 years of hire
- Oregon Registered Nurse License upon hire
- 2 years of acute care hospital or community-based experience
- 1 year of experience with electronic medical records or documentation systems
- 1 year of home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience
