Job Summary
RN Care Manager in the acute care setting provides screening for post-acute needs, transition planning, care coordination, education, community care referrals, and care planning in accordance with regulatory requirements. Collaborates with the continuum care team to monitor and reassess care management needs; utilizes electronic medical records for documentation; supports patients through discharge planning and care coordination. Multiple FTEs and shifts available, with Part-Time and Full-Time options.
Required Qualifications
- Nursing degree/diploma required upon hire
- Bachelor's degree in Nursing or higher within 3 years of hire
- Oregon Registered Nurse License upon hire
- 2 years of acute care hospital or community-based experience
- 1 year of experience with electronic medical records or documentation systems
- 1 year of home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience
