Justice Involved Lead Care Manager

Job Summary

Conduct field-based, telephonic, and facility-based outreach to justice-involved members transitioning from incarceration, completing comprehensive assessments for medical, behavioral health, and social needs. Develop individualized Care Management Plans and coordinate physical health, substance use treatment, housing, and transportation services while collaborating with healthcare providers, correctional facilities, and community organizations. Assist members with community reintegration, benefits enrollment, and medication continuity to reduce emergency department utilization and support long-term stability. Maintain accurate documentation in compliance with DHCS, Medi-Cal, and HIPAA standards while providing trauma-informed, culturally responsive care to diverse populations.

Required Qualifications

  • High school diploma or GED
  • Minimum of one (1) year of experience in case management, care coordination, behavioral health, healthcare outreach, reentry services, or related field
  • Valid California driver's license
  • Reliable transportation

Desired Qualifications

  • Associate degree or higher in social work, psychology, healthcare, criminal justice, human services, or related field
