Job Summary
Provide case management with crisis intervention, discharge planning, and group or individual work to support clients' mental health and symptom management. Initiate referrals to next-level care, coordinate with physical health providers, and facilitate placements including pre-placement visits and transportation. Assist clients in navigating the justice system by attending court, visiting incarcerated individuals, and acting as a liaison with probation, parole, and jails. Complete accurate documentation to meet billable requirements while maintaining confidentiality. Shift coverage includes part of the on-call team, and field-based services are provided as needed.
Required Qualifications
- Master's degree with two years of experience in social services; or bachelor's degree with four years of experience in social services
- Valid CA driver's license and current auto insurance per contract requirements required
- Cultural competency understanding required
Desired Qualifications
- Experience providing mental health and/or substance use disorder services preferred
- Bilingual preferred
- Knowledge of Microsoft Office and EHR software preferred
