Job Summary
Conduct monthly on-site visits for Medicaid (ALTCS) clients, verifying care delivery, assessing safety, and identifying service gaps. Perform Private Duty and VA assessments covering mobility, cognitive ability, and home conditions while managing a high-volume caseload. Coordinate staffing coverage, match caregivers to client needs, and support full lifecycle case management including service plan development. Complete mandatory Monday in-office planning and documentation, ensuring all visit notes, care plans, and logs are audit-ready. Serve as the primary field communication link for clients, caregivers, and operations to report real-time escalations regarding hospital admissions or safety risks.
Required Qualifications
- High school diploma
- Valid driver's license
- Reliable transportation
- Ability to obtain Level One Fingerprint Clearance Card
- AHCCCS-required credentials
- Experience in home care or healthcare operations
- Care coordination or compliance experience
- Strong knowledge of Arizona Medicaid (ALTCS) AHCCCS requirements
- Strong knowledge of Home and Community-Based Services (HCBS)
- Strong organizational and multitasking ability
- Excellent communication and interpersonal skills
- Critical thinking and problem-solving ability
- Ability to work independently in a field-based role
- Strong documentation and compliance awareness
- Ability to travel regularly throughout service areas
- Monday in-office attendance
