Job Summary
Serve as the primary point of contact for residents and families on the memory care unit by providing education, answering questions, and addressing concerns throughout their stay. Collaborate with nursing, therapy, social services, admissions, and other interdisciplinary team members to ensure resident needs are met. Assist with admission and discharge planning, including coordinating services, documentation, and follow-up care. Facilitate communication between residents, families, physicians, and the team to promote continuity of care. Conduct resident and family satisfaction rounds and assist in resolving concerns promptly. Support care conferences and participate in interdisciplinary meetings to advocate for resident-centered care. Maintain accurate documentation and ensure compliance with all facility policies and applicable federal and state regulations. Assist with resident engagement initiatives and programs that enhance the overall resident experience. Perform other duties as assigned to support exceptional resident care and customer service.
Required Qualifications
- Must be a Licensed LPN in the state of Maryland
Desired Qualifications
- Previous experience in a Skilled Nursing, Rehabilitation, Long-Term Care, or Assisted Living setting preferred
- Excellent communication, customer service, and interpersonal skills
- Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment
- Ability to build positive relationships with residents, families, physicians, and interdisciplinary team members
- Proficiency in Microsoft Office and the ability to learn electronic medical record (EMR) systems
- Knowledge of resident rights, HIPAA, and healthcare confidentiality standards preferred
