Job Summary
Assess each person's hopes and desires to provide supports that facilitate goal achievement while developing positive relationships with natural supports. Assist individuals in participating fully in their community, accessing agency services, and utilizing personal advocates. Provide support for emotional, physical, and medical needs, educate on facility policies, and implement individual plans with accurate documentation. Facilitate person-centered relationships promoting self-determination, supervise work settings, and maintain safe environments through safety drills and equipment inspections. Substitute for specialists during vacations or vacancies, drive and travel as required, and assist with daily home operations including housekeeping and meal preparation. Sleepovers at residential settings may be required.
Required Qualifications
- High School Diploma
- Valid Driver's License - USA
- Experience working with children or adults (dependent upon population served) with a mental health diagnosis, an intellectual/developmental disability, or substance use disorder
