Job Summary
Provide work placement and community integration support for individuals in Employment Services while they are in the program, at community work sites, and in the community. Monitor and ensure the health, safety, rights, and dignity of participants to encourage independence, and maintain all required community job placements consistent with employers' business practices. Facilitate natural supports, promote positive peer relations, and complete case management duties including advocacy, team planning, and timely documentation of Career Plans, Case Notes, and Support Strategies. Use your own dependable vehicle to transport individuals and assist with logistics for off-site placements, including scheduling and work performance training. Provide job coaching support as needed, including evening and weekend hours, while maintaining confidential files and attending regular supervision meetings and staff trainings.
Required Qualifications
- Must have a driver's license and a clean driving record
- Must own an appropriate vehicle for transporting individuals
- Must be able to lift and support the body weight of any program individual with the assistance of another employee
- Must meet Massachusetts Criminal Offender Record Information (CORI) background check requirements
