Job Summary
Provide on-site job coaching and advocacy for individuals in the intensive and follow-along phases of Supported Employment. Conduct observational assessments to assist supported persons in learning job tasks, while maintaining case files and completing accurate documentation of SAP plans, case notes, monthlies, and DDPs. Prepare monthly documentation for ACCES and OPWDD and act as a liaison between the person supported, family, and employer. Travel via public transportation and roadways to designated sites, attend all staff meetings and training requirements, and offer overall assistance as assigned. Full-time role with Monday through Friday hours from 9am to 5pm, with flexibility needed.
Required Qualifications
- High School Diploma or equivalent
- Clean, valid New York State driver's license
- Mon- Fri 9am-5pm (flexibility needed)
Desired Qualifications
- Associate's Degree in Psychology, Sociology, Education or related field preferred
- Minimum of one year experience providing vocational or pre-vocational services to supported persons preferred
- Complete (within first year of employment) twenty-four (24) hours of OPWDD's Innovations in Employment Supports training
