Job Summary
Coordinate with individuals and their Circle of Support to identify employment preferences, barriers, and person-centered planning needs. Conduct evaluations, provide training, and offer ongoing support to maintain employment within the community. Establish relationships with employers and co-workers to build natural support networks and address work performance barriers. Monitor and complete required Medicaid Waiver or ECF documentation to ensure compliance with state standards. Maintain records tracking hours worked, wages, and progress while participating in department meetings to streamline organizational goals. Complete necessary trainings for DDA, ECF, DCS, and MAPs programs, including Virtual Community Resource Map and Travel Trainer certifications. Promote high satisfaction among customers by implementing proactive action plans and addressing concerns. Operate a motor vehicle with valid Tennessee license and liability insurance.
Required Qualifications
- High School Diploma or equivalent
- Has completed or will complete TRN Job Coaching Training
- 12-hour shadow-training requirement
- Operation of a motor vehicle
- Proof of a valid driver's license
- Ability to maintain automobile liability insurance as required by state law in Tennessee
- Ability to qualify as a driver under MillarRich, LLC insurance guidelines
- Flexibility in assigned working hours
- Time management and follow-up skills with ability to plan, organize and prioritize a large workload
- Ability to handle several tasks simultaneously
Desired Qualifications
- Bachelor's degree in a human service field
- One-two years of prior experience in a position of similar scope
- Knowledge of de-escalation techniques and/or behavior analysis
- The ability to effectively communicate through verbal and written mediums
- The ability to observe, ask questions, listen actively, and give and accept feedback effectively
