Job Summary
Supports development of Individual Habilitation Service Plans and tracks individual progress toward goals in Staff Action Plans. Segments job duties into specific steps to facilitate learning, monitors performance levels to determine support fading, and ensures work adheres to industry standards and time frames. Serves as a liaison between employees and employers to facilitate worker-supervisor relationships, maintains accurate records of coaching services, and documents recommendations to overcome employment barriers. Responds to crises in accordance with policy and training, making all decisions based on agency vision and values. Requires a valid NYS driver's license, reliable transportation, and ability to lift 35 lbs.
Required Qualifications
- High School diploma or GED
- six (6) months of employment experience
- Good math skills
- Good communication skills
- Valid NYS driver's license
- Reliable means of transportation
- Ability to bend, push, pull and lift a minimum of 35 lbs
