Job Summary
Clinical Social Worker Health Care Facility Surveyor needed to support a Quality Improvement Consulting Company, working nationwide with CMS requirements. Requires a Master’s in Social Work, CMS Long-Term Care Basic Training, SMQT, and an LCSW license, with at least 2 years’ surveyor experience accrediting long-term care facilities. Travel up to 75% is required. Full-time role based in Pennsylvania with Monday–Friday schedule; salary $72,000–$80,000 per year plus comprehensive benefits and travel perks (airfare/hotel, travel expenses covered, 401(k) with match, HSA, health/dental/vision, life and disability insurance). Required Qualifications
- Master's degree in Social Work
- CMS Long-Term Care Basic Training
- Surveyor Minimum Qualifications Test (SMQT)

- Current licensure to practice as an LCSW
- US Citizenship
- Ability to travel up to 75% of the time
