Required Qualifications
- Master's degree in Social Work

- passed the Surveyor Minimum Qualifications Test (SMQT)
- at least two (2) years of recent experience as a surveyor accrediting or certifying long-term care facilities
- current licensure to practice as an LCSW
- ability to travel up to 75%
