Job Summary
Clinical Social Worker Health Care Facility Surveyor in Oklahoma responsible for CMS surveys of long-term care facilities, requiring a Master's degree in Social Work, CMS Long-Term Care Basic Training, and SMQT. Must hold and maintain LCSW licensure, have at least two years of surveyor experience accrediting or certifying long-term care facilities, and be able to travel up to 75%. The role supports internal and external customers nationwide in a Quality Improvement Consulting environment; offers full-time schedule (Monday–Friday) with extensive benefits including paid travel, retirement plans, health and life insurance, HSA, and training opportunities. Required Qualifications
