Job Summary
Clinical Social Worker Health Care Facility Surveyor role supporting CMS Quality Improvement initiatives. Requires a Master’s in Social Work, CMS Long-Term Care Basic Training and SMQT, 2+ years as a surveyor for long-term care facilities, current LCSW licensure, and ability to travel up to 75%. Required Qualifications
- Master's degree in Social Work
- CMS’s Long-Term Care Basic Training and passed the Surveyor Minimum Qualifications Test (SMQT)
- At least two (2) years of experience as a surveyor accrediting or certifying facilities serving residents of long-term care facilities
- Current licensure to practice as an LCSW
- U.S. Citizenship
- Ability to travel up to 75% of the time
