Job Summary
Clinical Social Worker Health Care Facility Surveyor role requiring a Master's in Social Work, CMS Long-Term Care Basic Training and SMQT, and at least two years of surveyor experience in accrediting long-term care facilities. Must maintain LCSW licensure, demonstrate independent decision-making to direct the survey process, and travel up to 75% regularly. Required Qualifications
- Master's degree in Social Work
- CMS Long-Term Care Basic Training and SMQT

- U.S. Citizenship is required
