Job Summary
Clinical Social Worker Health Care Facility Surveyor in Arizona. Role requires an MSW, CMS Long-Term Care Basic Training, and SMQT, with at least two years of surveyor experience accrediting or certifying long-term care facilities. Must maintain active LCSW licensure, demonstrate independent decision-making in directing survey processes, and prioritize tasks. Travel up to 75% is required. Required Qualifications
- Master's degree in Social Work
- CMS Long-Term Care Basic Training
- Surveyor Minimum Qualifications Test (SMQT)

- U.S. Citizenship
- Ability to travel up to 75% of the time
