Jeugdmaatschappelijk werker FACT Jeugd Noord

Conduct intensive, restorative outreach and home visits for young adults aged 14 to 24 with severe psychiatric disorders, acting as the primary case manager and coordinator for their treatment plans. Lead multidisciplinary mini-teams of three to four professionals to manage caseloads, prevent hospital admissions, and address systemic barriers in education, employment, housing, and relationships. Coordinate with external partners including the Crisis Service, Child Protection, and GGD within Amsterdam Centrum and Noord while maintaining independent scheduling and crisis scaling with the team. Work on a 32-36 hour contract at the FACT Jeugdteam Noord on the NDSM-terrein, supporting flexible Assertive Community Treatment for youth with serious mental illness.
