Job Summary
Provide crisis and interim counseling, psychoeducation, care coordination, advocacy, and transportation to individuals and families involved in the Jail Diversion Program. Conduct initial intake meetings to assess mental health, substance use, and risk levels, then develop and implement person-centered treatment plans with participants. Facilitate supported referrals to community resources and emergency services as needed, while collaborating with supervisors, probation officers, and multidisciplinary teams to monitor progress and adjust workflows. Maintain accurate documentation in electronic health records and participate in regular case reviews and supervision. This role requires travel throughout the Blackstone Valley and includes a Monday through Friday 10am to 6pm schedule.
Required Qualifications
- Master's degree
- at least one year minimum experience working with individuals in a crisis or community-based support setting
- Ability to develop and maintain satisfactory working relationships with participants and community providers
- High energy level

- Must adhere to all professional ethics
- Ability to communicate effectively verbally and in writing
- Must have basic computer skills
- Ability to read and speak English

- Must be able to perform each essential duty satisfactorily
- Must be able to travel throughout the Blackstone Valley
