Job Summary
Personal Outcomes Coach for ISLA program provides ongoing training and support to individuals with intellectual and developmental disabilities to help them achieve their individualized outcome goals per ISP. Responsibilities include delivering daily living skills coaching, maintaining detailed progress notes, ensuring compliance with funding guidelines, coordinating with guardians and service providers, developing and reviewing safety plans, and representing Sunnyhill with professionalism and a joyful, positive attitude while fostering a fun, caring environment. Flexible hours between 7 a.m. and 11 p.m. and opportunities for growth are offered; prior experience is a plus but training is available.
