Job Summary
Plan, develop, and implement social service programs while assisting with discharge planning and resident assessments. Conduct interviews to obtain social histories and create written plans of correction for survey deficiencies. Develop comprehensive care plans identifying resident needs and goals, then maintain accurate, descriptive progress notes. Participate in community planning and schedule in-service training for the department. Ensure confidentiality of all resident information and foster a calm, positive environment throughout the facility.
Required Qualifications
- Must be currently enrolled at an institution of higher learning
- Must be able to read, write and follow written and oral instructions in English
- Must be able to make independent decisions concerning above job duties
- Must possess the ability to deal tactfully with personnel, residents, family member, visitors, government agencies/personnel, vendors, and the general public
