Job Summary
Provide comprehensive primary and acute care to patients in a nursing home setting, diagnosing and managing acute and chronic conditions. Develop individualized care plans and collaborate with nurses, specialists, social workers, and families to coordinate patient care. Maintain accurate, timely, and compliant medical documentation while participating in care conferences and family meetings. Full-time or part-time schedule available for residents, fellows, and new graduates.
Required Qualifications
- MD or DO from an accredited medical school
- Active New York State Medical License
- Board Certified or Board Eligible in Internal Medicine
- Proficiency in adult medicine
- Proficiency in geriatric care
