Job Summary
Provide comprehensive primary and acute care to patients in a nursing home setting within a multi-specialty environment in Westchester County, NY. Diagnose, treat, and manage a wide range of acute and chronic medical conditions; develop, implement, and review individualized care plans; collaborate with nurses, specialists, social workers, and families to coordinate patient care; maintain accurate, timely, and compliant medical documentation; participate in care conferences and family meetings. Required Qualifications
