Job Summary
Engage Guests with complex vulnerabilities, including those navigating homelessness, substance use, and mental health challenges, using a respectful, trauma-informed approach. Provide intensive, housing-focused case management tailored to each Guest's goals, emphasizing autonomy and social determinants of health. Coordinate services across internal teams and external systems to ensure continuity of care, while supporting crisis management through de-escalation and safety planning. Maintain accurate documentation in the electronic case management system and supervise BSW student placements as required. Participate in interdisciplinary team meetings to contribute to care planning. Work flexible hours including days, evenings, and weekends.
Required Qualifications
- Degree in Social Work
- Registration with the Newfoundland and Labrador Association of Social Workers (NLASW)
- Minimum 2 years' experience in case management or a related support role
- Experience with marginalized or high-acuity populations
- Knowledge of local and provincial community services and housing resources
- Clear Record of Conduct
- Vulnerable Sector Check
