Job Summary
Provide outpatient geriatric-focused internal medicine care to approximately 18 patients daily in an independent private practice in Thornton. Conduct patient visits under APP supervision while managing a 1:6 call schedule. Utilize Allscripts EHR for documentation and care coordination. This role supports the PHPprime network, which offers wrap-around services including care coordination, risk adjustment coding education, and population health management to enhance practice success.
Required Qualifications
- Board eligible/ board certified internal medicine
- Supervisory Skills
- interest in working with advanced practice providers
- APP supervision is required
- Call schedule 1:6
- EHR – Allscripts
