Job Summary
Provide comprehensive outpatient primary care for adult patients by diagnosing and managing acute and chronic conditions while performing preventive care, health screenings, and wellness visits. Coordinate care with specialists and integrated care teams, collaborate with physicians and APPs, and maintain accurate documentation using EMR systems. Participate in quality improvement and value-based care initiatives with shared call coverage depending on practice needs. Board certified or eligible physicians with Connecticut licensure are encouraged to apply to this community-focused role affiliated with Yale School of Medicine.
Required Qualifications
- Board Certified or Board Eligible in Internal Medicine
- Eligible for Connecticut State medical licensure
- Active or eligible DEA registration
