Internal Medicine or Family Medicine Physician (Outpatient)

Provide outpatient care across pediatric, adult, and geriatric populations by diagnosing and managing chronic and acute conditions, conducting physical exams, preventive visits, immunizations, and routine screenings. Coordinate care with specialists, behavioral health teams, and care managers while maintaining accurate, timely documentation within the EMR. Participate in quality improvement initiatives and population health programs to foster strong, trusting relationships with patients and their families.
