Job Summary
Intern - Population Health works within the CHN population health department to support care coordination, quality improvement, and health equity initiatives for patients across NYC FQHC sites. The role includes telephonic liaison work between patients and care teams, clinical shadowing across interdisciplinary teams, referral coordination, portal outreach, and data support, with core activities spanning remote and in-person engagement, daily activity tracking, and a capstone project. Interns will contribute to outreach, preventive care support, and operational workflows focused on health equity, patient access, and measurable quality improvement. Expected responsibilities include remote outreach like portal sign-ups and referral outreach, maintaining HIPAA-compliant practices, and participating in capstone development and leadership presentations, while gaining hands-on experience in whole-person care within an FQHC setting.
Required Qualifications
- Enrollment in a health-related program or related professional coursework
- Strong communication and outreach skills
- Ability to work in both remote and in-person formats
- Basic familiarity with HIPAA and patient confidentiality
