Interim Housing Case Manager

Job Summary
Manage a caseload of approximately 35 participants by performing Individual Service Needs Assessments and collaborating to develop Individual Service Plans focused on housing. Meet with participants to implement these plans, create action plans for goal accomplishment, and provide linkage services for housing, income, primary care, substance use treatment, behavioral health, legal, employment, and other resources. Report progress to Program Leadership, track attendance at scheduled appointments, maintain accurate participant files and records in electronic systems, and assist with planned exits while adhering to agency, state, and funder regulations. This fully onsite role requires consistent in-person engagement and the ability to quickly assess and respond to crises, including medical and psychiatric emergencies.
Required Qualifications
- High School Diploma + 4 years' experience or AA degree in similar field + 3 years' experience, or CHW Certificate/CADC + 2 – 4 years' experience, or BA/BS degree in similar field + 2 – 4 years' experience, or MA/MS degree in similar field + 1 year experience
- Associate degree in psychology, counseling, social work, or another related field and 3 years of relevant work experience
- CCSF Community Health Worker Certificate and 2 – 4 years of relevant work experience
- High school diploma/GED and 4 years of relevant work experience
- Experience working in integrated service teams using person-centered harm-reduction and trauma-informed approaches
- Background working with unhoused individuals
- Effective communication skills (written and oral)
- Excellent organizational and time management skills
- Strong administrative and organizational skills
- Must possess excellent charting, recordkeeping, and professional writing skills
- Knowledge of Microsoft Office programs and experience documenting services into electronic record systems
- Must be able to problem solve and quickly assess/respond to a variety of situations, including medical emergencies, psychiatric emergencies, conflict resolution and de-escalation interventions, as well as other types of crises
- Operate with cultural humility in a diverse program and staff population
- Possess a knowledge of and sensitivity to adults with substance abuse, domestic violence, incarceration, HIV/AIDS, mental health including co-occurring disorders, and history of trauma
- Must secure fingerprint image screening and annual TB screening
- Must be able to meet physical requirements of the position, which may include lifting up to 30 lbs
