Intercept Specialist

Job Summary
Conduct intensive home-based case management for 4-6 families, delivering an average of three face-to-face sessions weekly to safely reunify youth and maintain them in home environments. Deliver ongoing risk assessments, safety planning, and therapeutic interventions while collaborating with schools, courts, and community agencies to coordinate comprehensive care. Maintain accurate documentation of all client activities, treatment plans, and clinical assessments, and provide rotating on-call after-hours support for crisis situations. Travel within the service area using a personal vehicle and attend required team staffing meetings for supervision and professional development.
Required Qualifications
- Bachelor's degree in Social work, psychology, or related human services field from an accredited college or university
- 1 year of cumulative experience with an undergraduate degree which includes working with children and families providing counseling, crisis intervention, intake and assessment, and case management

- Must maintain confidentiality & follow HIPAA and agency policies related to personal records & client records
- Specialists are expected to adhere to all ethical and legal guidelines, including confidentiality and mandated reporting
- Complete trainings and certifications as required by the Intercept program, licensing standards, and agency policies
