Job Summary
Link participants with medical, social, and educational providers to address identified needs and achieve goals. Conduct health, safety, social, and environmental assessments per established guidelines. Serve on work groups, community groups, committees, or task forces as requested. Maintain confidentiality regarding patient information and adhere to CareStar policies and procedures. This role requires a Bachelor's degree in a human service field and two years of direct service experience serving persons with disabilities. Candidates must possess reliable transportation, a valid driver's license, and the ability to travel. Required Qualifications
- Bachelor's degree in human service Field such as Social Work, Psychology, Sociology, Counseling, Gerontology or Nursing (or closely related field)
- Two year's direct service experience serving persons with DD/IDD or related disabilities
- Two years' experience in healthcare, habilitative/rehabilitative, residential, behavioral health setting (ICF/IDD), psychiatric inpatient hospital/facility, specialty clinic, special education department or similar setting
- Experience in completing assessments, service plan development and monitoring
- Effective organizational and communication skills
- Valid driver's license
- Car insurance as required by state law
- Ability to travel
- Willingness and ability to work flexible schedule to meet needs of families
Desired Qualifications
- Experience providing care management/coordination preferred
