Job Summary
Provide intensive, client-centered case management to residents in Permanent Supportive Housing using a Housing First and Harm Reduction approach. Develop trusting relationships, conduct psychosocial assessments, and create individualized service plans updated quarterly. Coordinate referrals to primary care, behavioral health, substance use treatment, and community resources while performing home visits, wellness checks, and accompanying clients to appointments. Respond to crises with trauma-informed intervention and de-escalation, collaborating with multidisciplinary teams and emergency responders as needed. Maintain accurate documentation in CHAMP, HMIS, and APRICOT while managing a caseload that varies by acuity and program staffing requirements. Work full-time, M-F at Long Beach or Wilmington locations with occasional evening or weekend flexibility.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field
- at least one year of experience working with individuals experiencing homelessness
- Two years of experience providing direct mental health services or intensive case management
Desired Qualifications
- Experience working with individuals experiencing homelessness, serious mental illness, substance use disorders, and chronic health conditions
- Knowledge of Housing First, Harm Reduction, trauma-informed care, and community resources
- Strong communication, organization, documentation, and time management skills
- Experience with electronic documentation systems (HMIS, CHAMP, APRICOT, or similar)
- Ability to work collaboratively while managing competing priorities in a community-based environment
- Ability to work occasional evenings or weekends as program needs require
