Job Summary
Facilitate evidence-based group therapy sessions for geriatric patients, addressing mental health conditions such as depression, anxiety, dementia-related behaviors, and adjustment disorders. Provide individual and family counseling as indicated in the treatment plan, while completing psychosocial assessments and contributing to multidisciplinary treatment team planning. Develop, implement, and update individualized treatment plans in collaboration with the patient, family, and care team. Document all services provided in the electronic health record in a timely and accurate manner, ensuring compliance with facility policies, state regulations, and payer requirements. Participate in daily or weekly treatment team meetings, providing updates on patient progress and recommendations. Coordinate care with internal staff and external providers, assist with discharge planning, and provide crisis intervention as appropriate.
Required Qualifications
- Master's degree in Social Work, Counseling, Psychology, or related field
- Current state licensure as LCSW, LPC, LMFT, or equivalent

- Experience in geriatric behavioral health
- Knowledge of age-specific issues, therapeutic modalities, and community resources for the elderly
- Strong organizational, communication, and documentation skills
- Ability to work effectively as part of an interdisciplinary team
