Job Summary
Hybrid Intensive Housing and Health Case Manager delivering housing case management and health navigation for participants using a client-centered, trauma-informed approach. Responsibilities include regular home visits, accompanying participants to medical and benefits appointments, coordinating move-ins/lease renewals/inspections, submitting housing paperwork to AFC within deadlines, collaborating with the Behavioral Health Specialist on psychosocial assessments, and connecting participants to public benefits (e.g., SNAP, SOAR) and community resources. Ensure compliance with funder documentation requirements (IDHS, HOPWA) and maintain thorough, up-to-date participant files across databases. Skills emphasize relationship-building, crisis prevention/intervention, harm-reduction philosophies, and coordination of housing stability with medical care and treatment plans.
Required Qualifications
- High school diploma or equivalent
- 3+ years of case management experience
- Valid driver's license and access to a personal vehicle
- Comfort navigating Chicago by personal vehicle and meeting participants in their homes and community settings
