Job Summary
Write and implement plans of care approved by consulting agency clinicians, including contact notes, monthly reports, and discharge summaries. Maintain frequent communications with Family Service Workers to ensure case plan compliance and provide in-home family support during times that work for the family. Assist families in finding resources for housing, transportation, substance abuse treatment, and counseling while helping caregivers recognize children's needs and implement strategies for growth. Create crisis plans, respond to crisis calls, and provide transportation when required. Participate in meetings, conferences, and trainings as assigned, and testify in court when subpoenaed. Ensure Family Assessments are completed timely per IFBS policies and coordinate services with schools, Early Intervention services, and other providers. Implement short-term interventions to prevent DCF involvement or reunify children with their family.
Required Qualifications
- Bachelor's Degree in a related field
- Valid driver's license and reliable transportation
- Excellent writing skills
Desired Qualifications
- Two (2) years of experience preferred
