Intensive Community Based (ICB) Case Manager

Job Summary
Intensive Community Based Case Manager provides high-intensity, community-based services to children and youth aged 10-18, requiring multi-systemic coordination to remain out of hospital/juvenile services. Works with Intensive Community Based Therapist to implement individualized treatment plans, deliver strength-based services, teach coping skills to stabilize crises, and prevent out-of-home or inpatient psychiatric hospitalization. Performs assessments (e.g., PHQ9, AUDIT) and coordinates care across schools, juvenile justice, child welfare, primary care, and community organizations; conducts home, school, and community visits; supports Care Navigation, LOC monitoring, crisis prevention/diversion, resource linkage, and family support; ensures HIPAA/compliance and robust documentation.
Required Qualifications
- Bachelor’s degree in social work, Psychology, Human Services, or related field strongly preferred (or equivalent experience per state regulations).
- Experience working with children, adolescents, and caregivers experiencing high behavioral health needs.
- Knowledge of community resources, case management practices, and cross system‐ collaboration.
- Valid driver’s license, insurance, and reliable transportation.
