Job Summary
Intensive Case Manager role at Affordable Living for the Aging requiring experience with the homeless, expertise in mental health, and a Bachelor's or Master's degree. Responsible for managing cases, assisting clients with health services, and providing support for independence and well-being.
Required Qualifications
- Bachelor's or master's degree in social work or mental health related field
- At least one year of experience with homeless population
- Experience providing direct mental health or intensive case management services to homeless individuals
- Experience working with clients with mental illness, chronic health issues, and substance use disorders
Desired Qualifications
- Experience with the homeless population
- Expertise in outreach and engagement strategies
- Proficient knowledge of managed care organizations, Medicare, and Medi-Cal
- Excellent interpersonal skills and sensitivity to working with culturally diverse populations
- Well-organized and able to comply with documentation and reporting requirements
- Ability to maintain professional boundaries with clients and effectively manage time
