Job Summary
Intensive Case Manager position in Los Angeles County focusing on housing, mental health, and substance abuse treatment linkages to minimize recidivism. Core duties include interviewing and assessing participant needs, conducting wellness checks and crisis management, developing individualized treatment plans, assisting with public benefits advocacy, coordinating housing and mental health resources, coordinating with staff and external partners (probation, psychiatrists, public defenders, primary care providers), attending inter-agency case meetings, maintaining concise case records, and completing accurate documentation per funding guidelines. Requires use of a company and personal vehicle to accompany clients to appointments and jail visits. Required Qualifications
- Bachelor’s degree in behavioral health or social science and one year of experience providing outreach, substance abuse counseling
- The equivalent of a high school diploma and three cumulative years of experience of administrative responsibility in providing outreach, substance abuse counseling, reentry services, and case management within a culturally diverse population
- Knowledge on how to administer assessments, substance abuse counseling, reentry services, and case management within a culturally diverse population, preferably with both men and women
- Excellent organizational skills and attention to detail
- Excellent written and oral communication skills
- Extensive knowledge of non-profit organizations
- Proficient in Microsoft Office Suite or similar software
- Required to have car insurance, vehicle, and valid driver’s license
