Job Summary
Engage clients in implementing Individual Recovery Plans based on their self-identified goals, strengths, and barriers. Provide support and training while linking clients to community services to address identified needs. Conduct regular community visits to monitor progress, decrease hospitalizations and homelessness, and increase housing stability and employment. Collaborate with doctors, residential providers, and other services to deliver wraparound support. This role works with clients' doctors, residential providers, and other services involved to provide wrap around support.
Required Qualifications
- Bachelors' degree in Psychology, Social Work, or a related field
- Professional and courteous communication (verbal and in writing)
- Knowledge of and sensitivity to culturally diverse populations
- Valid Georgia Driver's License
- Clean Motor Vehicle Record (MVR)
- Clean Criminal Background Check (no conviction or credible evidence of abuse, neglect, or sexual exploitation-child or adult)
