Intensive Care Manager, Overflow (Gallatin, TN)

Provide continuous treatment team care management services to children and youth with serious emotional disturbance or severe mental illness, their families, and other team members’ consumers. Mentor new care managers on basic skills. Develop and implement individualized service plans and crisis plans with measurable goals; assist with referral and linkage to community resources and advocate for consumers. Ensure timely follow-up upon hospital discharge and Diversion Services within Agency standards; coordinate consumers’ psychiatric, medical, and environmental needs; maintain timely documentation; participate in team meetings; educate consumers and involved collaterals about psychiatric diagnosis and treatment options; build and maintain relationships within the family unit and supports; uphold Joint Commission competencies and ensure progress notes clearly link to care plan goals.
